MDS COORDINATOR, RN, Registered Nurse - Full-time
University Place, a part of Franciscan Ministries, is looking for a full-time MDS Coordinator, RN, Registered Nurse to join our growing team! We are a vibrant senior living community in West Lafayette, IN. We offer Assisted Living, Memory Support and Skilled Nursing/Long-term Care. Additionally, the onsite Rehabilitation Clinic offers inpatient and outpatient physical, occupational, and speech therapies, and state-of-the-art programs designed to enhance the quality of life for residents and patients in the local community.
Responsibilities:
Transmit MDS data to the state as designated by the Coordinator- RN with current Indiana nursing license in good standing
- Recent experience as an MDS coordinator in a skilled nursing facility
- Excellent assessment and clinical skills
- Should be familiar with all aspects of the MDS including the assessments, submission requirements, etc.
- Must be able to work in collaboration with the interdisciplinary team in assuring that the MDS is completely timely and submission is done within appropriate time frames
- Should be computer literate (PCC a Plus)
- Must be able to work in collaboration with the Department Management team and Administrator
- Should be able to work scheduled hours
- Review MDS/RAP for accuracy and consistency
Required skills and qualifications:
Graduate from an accredited school of Nursing- Valid licensed in the state of Indiana
- At least 2 years experience with Long-term Care MDS
