WOUND CARE NURSE PRACTITIONER

POSITION SUMMARY OF WOUND CARE NURSE PRACTITIONER:
Our Nurse Practitioners provide care that reflects an understanding of individual cultural and religious diversities and needs as well as recognition of patient’s rights, and creates an atmosphere of courtesy and respect towards patients, families and staff. We provide and manage continuous medical care and service to patients under direct consultation with the Medical Director of SCH Wound Care/Hyperbaric Center, and in collaboration with the private medical attending as indicated. We respond to the requests of the nursing staff as required to ensure patient care and safety and perform medical examinations, evaluations, assessments, diagnoses, and treatments to outpatients in the wound care center

• Prescribes pharmaceuticals and other medications and treatment regimens as appropriate to assessed medical conditions
• Refers patients to specialists and to relevant patient care components as appropriate. Consults with medical Director and/or attending whenever patient’s condition warrants
• Provides inpatient wound care consults
• Provides back-up support for hyperbaric patients
• Participates in weekly inpatient skin care rounds

WOUND CARE NURSE PRACTITIONER REQUIREMENTS:

• Education: Degree from an accredited Nurse Practitioner program.
• Licensure/Certification: NYS Certified RN - NP (Adult or Family Nurse Practitioner). Trained in adult medicine. ACLS & BLS Certified, Wound Care Certified (WOCN) required
• Skills, Knowledge or Abilities: Experience in patient case management required; hospital experience preferred. Experience in similar setting preferred. NP must be certified in wound care (WOCN) and maintain current collaborative practice agreement with the Medical Director of the center. Completion of accredited hyperbaric introductory course required.
