Two day a week position (16 hours a week)

Total Family Medical is a small BUT busy rural health care family practice that also offers mental health services two days a week. Our LCSW works very closely with our Mental Health Nurse Practioner/Medical Psychologist to faciliate the best outcomes for our patients.

We firmly believe that it is the combination of counseling and medication management (when needed) that produce the most positive outcomes!

We are looking for a compassionate Social Worker to act as advocate and help people foster their capacities and coping mechanisms in order to improve and better their daily lives and experiences, within specified standards and timescales.

Total Family Medical is a Medicaid-focused medical practice based in Robert, LA. Our services include, comprehensive general and preventive medicine to adults and teens, women’s health care, and behavioral health. What You’ll Be Doing

The LCSW is responsible for providing and coordinating all aspects of patient psychosocial needs, and will work within an interdisciplinary team to ensure appropriate treatment.

The successful candidate will be able to:

· Conduct clinical psychosocial assessments

· Implement and manage treatment plan

· Provide therapeutic counseling sessions for patients

· Effectively respond to patients, family members in a positive and timely manner

· Collaborate with mid-level provider managing medication treatments for patients

· Communicate information to providers and other medical professionals

· Perform some case management duties when needed

· Perform any and all other duties as assigned

What We’re Looking For

· Master’s degree in Social Work required

· Licensed Clinical Social Worker in Louisiana required

· 2+ years of experience in the behavioral health services field preferred

· Excellent PC skills, specifically with Microsoft Office Suite

· Strong organizational skills and ability to multi-task

· Ability to thrive in a busy clinic setting and maintain a passion to help those in need

· Excellent verbal and written communication and interpersonal skills required

· Work independently, requiring minimal or no oversight and guidance

Responsibilities

• Conducts initial "intake" 30 minute appt to ensure patient acceptance into practice and is not suffering with emergent psychiatric need
• Conducts 50 minute ongoing therapy sessions for patients in conjunction with medication management via the Mental Health Nurse Practitioner.
• Plan, coordinate, manage and implement community resources to help clients deal with difficulties and overcome their current concerns.
• Works with patient to set tailored measurable goals
• Monitor and evaluate clients’ progress and modify treatment plans accordingly
• Offer information and counseling on the best course of action during sessions
• Maintain accurate records and report on clients’ status
• Skills
• Proven work experience as a social worker
• Working knowledge of social theories and practices
• Social perceptiveness and empathy
• Ability to build and maintain professional helping relationships
• Ability to relate and communicate with diverse population and groups
• Resilience along with ability to assess situations
• BS degree in social work (BSW), psychology or sociology
• Must be an LCSW or CSW

We're searching for an LCSW to do initial psychosocial intakes, n going 1:1 couseling, and to to coordinate medical and other services to members of our close-knit community. This position is a great opportunity to work with a diverse patient population while collaborating with our other providers. The successful candidate must have a strong understanding of psychological disorders, good communication skills and an ability to find common ground during difficult conversations.

• Learn and apply the theory, methods, and skills of therapy.
• Take care of yourself as a therapist.
• Provide therapy to a wide range of patients.

Pay: $30.00 - $42.00 per hour
