Telecommute 4: OHCW Care Manager-Licensed Social Worker or Registered Nurse Job at Area Agency on Aging District 7, Inc.
SUMMARY: Home office based position which provides care management services to individuals with disabilities age 59 years and younger enrolled on the Ohio Home Care Waiver. Performs in person eligibility assessments in a variety of settings and develops a person centered service plan to meet the individual’s needs. Monitors formal and informal services provided to individuals to ensure their health and safety in a home based care setting. Maintains contact with enrolled individuals through phone calls and home visits and responds to changes in the individual’s care needs by adjusting authorized services and the person centered service plan. Training will be provided through a combination of in person sessions in our West Union office, webinars, and mentoring sessions. After training is completed, this will be a telecommuting position serving clients in Hamilton and Clermont counties.
POSITION QUALIFICATIONS:
1. Must be either a Registered Nurse (RN) who possesses a current, valid and unrestricted license with the Ohio Board of Nursing with one (1) year paid clinical experience in Home and Community Based Service, or a Licensed Social Worker (LSW) or licensed independent social worker (LISW) who holds a current, valid and unrestricted license to practice issued by the Counselor, Social Worker, & Marriage & Family Therapist Board in the State of Ohio with one (1) year paid clinical experience in Home and Community Based Service.
2. Possess knowledge and skills necessary to provide the level of care management required by the individual.
3. Possess an understanding of the community service delivery system and the ability to utilize the community’s resources.
4. Ability to effectively communicate with individuals with disabilities and their support systems in person and via telephone or other electronic communication formats.
5. Ability to assess the health, hygiene, and safety of the consumer and his/her environment.
6. Ability to problem solve and work with individuals and their care teams to resolve ongoing issues and concerns.
7. Ability to evaluate individual’s support needs and implement services in a cost effective manner.
8. Ability to represent AAA7 in a professional manner in dress and conduct.
9. Possess general knowledge of Medicare and Medicaid.
10. Possess a good understanding of basic medical terminology.
11. Must display excellent interpersonal skills and ability to have collegial relationships with peer team members.
12. Must be flexible to provide on-call phone coverage after business hours in rotation with other team members (approximately 3 weeks per year).
Job Type: Full-time
Pay: $55,000.00 per year
