Job Description:
- Psychosocial support, hospice skills to include: assessment of patient/family needs, resource finding, bereavement support
- Charting System: EPIC
Required Certifications:
CNA: BLS certified
Education Required: CNA/Med Aide: High School diploma or GED equivalent required.

- Between Hospice and Home Health
