Compensation/Benefits: $45-$56K and a great benefit package that includes:

Responsibilities will include but are not limited to:
- Provide for medically related social service needs that are appropriate for the residents
- Assist residents in maintaining or improving their ability to manage their everyday physical, mental and psycho-social needs.
- Assist with conflict resolution as needed and aid with discharge planning when needed
- Solicit feedback from residents and/or family as to the quality of the care and services provided and report results to the appropriate departments
- Perform assessments for residents by completing the portions of the MDS and accurately chart results
- Initiate discharge planning for residents
- Participate in care planning process for residents
- Coordinate Family Council meetings with the Director of Nursing
- Responsible for monthly resident council meetings and raising any issues to appropriate manager
- Assist the Administrator and DON with documenting and investigating complaints
- Participate in management/leadership initiatives and attend in-service programs as required/needed.
What you need to be successful:
- Bachelors’ degree in social work or in a human services field that may include sociology, gerontology, special education, rehab counseling or psychology; AND one year of supervised social work experience in a health care setting
- Can do attitude with great people and teamwork skills
- Ability to understand and interpret issues related to long-term health care for individuals representing a variety of demographic background and care levels
- Long-Term care experience is strongly preferred
