What we are looking for:

Sage NeuroWellness is searching for an experienced Medical Case Manager to join our team. The perfect candidate for this position will have experience in coordinating care at nearly every stage of recovery and expertise with a variety of care needs including spinal cord injuries, brain injuries, multiple trauma, fractures, behavioral challenges, substance misuse, mental illness, ventilator/ trach dependency, etc.

What you will need to have:

• A degree in the medical field required.
• RN/BSN preferred
• Certified Case Manager (CCM) preferred
• MSW License preferred
• OTR preferred
• Minimum of 2 years’ experience as a Case manager
• Excellent verbal and written skills
• Willingness to travel to assignments
• Ability to work effectively in a fast-paced environment
• Strong analytical and organizational skills
• Excellent leadership, interpersonal and coaching skills
• Ability to effectively prioritize
• Flexibility, adaptability and initiative

What you will be doing:

• Provide direct Case Management services to thoroughly assess the medical, behavioral, vocational, and social needs of complex clients
• Provide a high level of treatment planning, implementation, and follow through to achieve the best possible outcome
• Establish a professional rapport with the client, family members/guardian, attorney, adjuster and medical providers to produce creative, efficient and cost-effective solutions
• Propose rehabilitation plans that coordinate with the client’s goals and objectives
• Report to the referral source and all interested parties in writing and verbally on a consistent and timely basis to facilitate collaboration and communication
• Maintain a high level of competency in Case Management including remaining up to date with the legal issues related to Michigan Auto No Fault and how those changes impact the industry and the clients served
