Statement of Purpose: This position develops, coordinates and provides social work services to community members who are faced with social, emotional and situational stressors related to dementia. This position is also responsible for providing education about Alzheimer’s disease, dementia, and brain health to individuals and families facing dementia and community members. Services are provided in accordance with organizational vision, mission and philosophy. This position promotes and supports collaboration through community programs and partnerships.

Reports to: This position reports directly to the Vice President of Home and Community Based Services.

Compensation: This position is hourly and non-exempt from overtime.

Major Tasks, Duties, and Responsibilities:

· Provides advocacy, education, assistance, support and referral services to community members in need. In the case of suspected abuse/neglect/exploitation, documents and reports where required.

· Provides both in person and virtual assessments of the clients functional and psychological needs and formulates an action plan, in partnership with the client. Facilitates adjustment in the action plan or services, as needed, to promote enhanced outcomes, ensuring all documentation is completed both timely and accurately.

· Gives presentations to the public on Sun Health’s MCN program. Represents Sun Health at marketing and outreach functions.

· Facilitates dementia care support groups.

· Facilitates referrals from the community through education and relationship building

· Develops and helps facilitate community education classes and presentations on topics related to dementia, caregiver stress, resources, brain health and other related topics. Personally teaches classes on areas of expertise and arranges for health professionals to teach classes on these topics as well.

· Manages, prepares, and maintains appropriate and accurate statistical data, client files, and program information. Oversees and assures the timely documentation and maintenance of appropriate statistics, accurate volunteer file maintenance, and compliance with all regulatory and/or accreditation requirements.

· Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.

Provides all customers/audiences of Sun Health with an excellent service experience by consistently demonstrating Sun Health behaviors (the 6 Cs) each and every day

Job Type: Full-time

Pay: $20.50 - $22.63 per hour
