Floating Options Counselor
Serving Blackford, Delaware, Fayette, Franklin, Grant, Henry, Jay, Madison, Randolph, Rush, Union, and Wayne Counties in east central Indiana
Join a team with a mission! LifeStream Services, east central Indiana's Aging and Disability Resource Center, is seeking a Managed Care Options Counselor. This position is responsible for referrals received through managed care contacts. Work involves completing assessments and coordinating services for individuals with chronic medical needs, pregnant women and behavioral health needs. Travel within LifeStream’s twelve (12) counties required.
Bachelor’s degree in social work, counseling, psychology, gerontology, a valid IN RN license or a Bachelor’s degree in any field with a minimum of two (2) years full-time, direct service experience with the elderly or disabled which includes assessment, service plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience. Must meet experience requirements outlined by the Division of Aging for A & D Medicaid Waiver, SSBG/Title III, CHOICE, PASRR, and TBI Waiver. Must be able to be certified in SHIP counseling and CRS-AD (CIRS-AD). LSW preferred.
