We are looking for a full time Health & Wellness Director.
POSITION SUMMARY: The Health and Wellness Director is responsible for managing the overall day-to-day healthcare operations of the community to ensure that resident’s healthcare needs are met. Ensures quality care as resident’s healthcare needs change. Supervises licensed nurses and other direct care staff within the community. Ensures that the Vision and Standards of Conduct are implemented and practiced within the Assisted Living Department.
Position Responsibilities
· Ensures provision of services to the residents and that the all Federal and State policies governing resident care are closely monitored and carried out.
· Assigns and directs work of subordinates; performance reviews, disciplining associates, addressing complaints and resolving problems.
· Participates in Resident Surveys and addresses resident and family complaints and grievances.
· Provides training, supervision, and monitoring of associates in the administration of medications per Title 22, to include auditing of medication records.
· Responds appropriately to resident emergencies by calling or contacting appropriate parties per protocol.
· Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by the physician.
· Assist Med Aides and Care Partners in the performance of their job requirements and assume caregiver and/or medication aide activities when needed.
· Coordinate departmental schedule and ensure adequate staffing in accordance with company standards, policies, procedures, budgets and state law.
· Ensure each assisted living staff member has completed the New Hire Orientation and are knowledgeable of the goals, policies, and procedures of the department. Provides training and education to Care Partners on an ongoing basis with classroom in-services, and situation specific training. Maintain the necessary training records of completion.
· Oversee and provide necessary training to department staff that ensures competent delivery of medications to residents, emergency procedures, record keeping, residents’ records and acceptable treatments. Ensure all care staff (AL and/or Wellington) complete state required training.
· Supervises the maintenance of resident charts and reviews documentation performed by staff.
· Conduct and coordinate the completion of an initial resident assessment is completed prior to each resident’s move-in date and assures that required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms.
· Provides ongoing assessment/observation of residents’ physical and psychosocial needs and coordinates with other departments to assure quality care. Documents changes in condition, notifies executive director, physician, and residents legally responsible party/family of resident’s condition.
· Ensures residents are prepared for physician appointments, prepares Physician Visit Form and reviews and updates resident medical records.
· Facilitates continuity of care for those residents receiving home health care, hospice services, and other third party healthcare related services.
· Participates in or leads meetings relevant to resident care issues, such as service plan meetings, involving appropriate parties as needed.
· Ensures in-house services such as podiatrist, doctor visits, labs, x-ray etc. are scheduled and followed through and acts as liaison with hospital personnel, physicians, community organizations and other health related service agencies to provide care to the residents.
· Coordinate prescription orders with doctors’ offices and pharmacy delivery of medications. Oversee central storage and delivery of medications.
· Complete all required data entry into the A.L. Advantage or other Care System to ensure maintenance of resident care information. Ensure that A.L. Advantage Care Profile and assessments are completed timely and all required documentation has been completed.
· Maintain listing of location of all oxygen tanks in the community. Ensure current list is provided to receptionist for use in emergencies.
· Maintains and adheres to monthly budget requirements, equipment, and supplies. Maintains monthly spenddown.
· Work with Executive Director to ensure implementation and maintenance of standards within the community and provide the Executive Director with information relative to the care needs of the residents and the department’s ability to meet those needs.
· As requested, participate in the Community’s Manager on Duty Program.
· Participate in daily “Stand-Up” meetings to communicate key issues within the department.
· Maintain a safe and secure environment for all staff, residents and guests, following established safety standards. Immediately report all hazardous conditions or equipment safety issues to the Executive Director.
· Demonstrate knowledge of infection control, i.e., adequate hand-washing, blood borne pathogens procedures, universal precautions, etc.
· Use Universal Precautions and follow established policies concerning exposure to blood/body fluids.
· Report occupational exposure to blood, body fluids, infectious materials and hazardous chemicals in accordance with the property's policies and procedures governing accidents and incidents.
· Perform fire and safety procedures when needed to assure resident/staff safety.
· Respond timely to resident pull cords/alerts as well as staff needs. Carry radio on person and respond as required.
· Communicates effectively and displays tact and friendliness when dealing with residents, families, visitors, co-workers, and supervisors.
· Support a positive and professional environment in the Community by adhering to guidelines in the employee handbook including dress code, time off requests and employee breaks.
· Maintain all required certifications/licensures and training as required by state law and company policy.
· Demonstrate flexibility in work schedule as evidenced by working holidays, evenings, weekends and additional shifts when necessary to ensure adequate coverage within the department.
· Performs other duties consistent with the position as assigned by the Executive Director.
· Encourage teamwork through cooperative interactions with co-workers and other departments.
· Aid in community marketing effort through demonstrated proficiency and willingness to conduct community tours and participation in all required marketing events.
· Ensure positive/friendly interactions with all potential residents, family members and referral sources.
SPECIFIC KNOWLEDGE, SKILLS, LICENSES, CERTIFICATIONS, ETC:
· Must be eighteen (18) years of age
· Education as required to obtain state nursing license (LPN/LVN, RN) and a minimum 2 years relevant experience. Must have current License.
· Current Basic CPR/First Aide Certification.
· Able to operate a personal computer and be proficient in MS Office programs (Word, Excel, etc.)
· Able to work with seniors and patiently interact with cognitively impaired individuals
· Able to be flexible, adapt and respond to change, make decisions in stressful situations and prioritize tasks and projects.
· Able to read, write and speak the English language and to interact and communicate effectively with employees, supervisors, physicians, health care professionals, residents, and their families
· Must be highly motivated and able to work independently
· Be mobile and able to perform the physical requirements of the job. Good physical health, verified by a health screening, including a chest x-ray or an intradermal test result not more than 6 months prior to employment are looking for caregivers for PM and NOC shift part-time and full-time.
